STATEMENT OF COMPLETION

We hope that you had a wonderful 2025-2026 school year!  To conclude this school year, please complete the following Statement of Completion.  This must be completed and submitted by June 30, 2026.  It indicates that your Upcountry Homeschool Association and South Carolina homeschool requirements have been completely satisfied during the past school year or that you are no longer homeschooling.  All Upcountry Homeschool Association members must have this statement on file for each year of membership.  

If you fail to submit a completed form your membership for the 2025-2026 school year, your membership will be considered void and your school year will not be in compliance with our association.

PLEASE ONLY COMPLETE THIS STATEMENT IF YOU HAVE:

• Completed your homeschool year of 180 days

• Enrolled in school during the 2025-2026 school year

• Moved out of state during the 2025-2026 school year

• Decided to school your children in another manner other than that represented by Upcountry Homeschool Association, LLC
